Indicative Module Description
The module is designed to provide students with the specialist theoretical and clinical practice skills to practice effectively in the specialty of older person care. With a strong clinical focus, the module aims to advance the student’s patient assessment skills and specialist nursing management of the older person with complex illness and or deteriorating health underpinned by evidence based care. The module also aims to further develop the nurse’s professional role with particular emphasis on patient-centeredness, patient safety, professional accountability and the requirement for effective communication inclusive of the multi-disciplinary team, the patient and their families.

Learning Outcomes:

At the end of the CPD the Student will:
1. Discuss specialist knowledge related to a range of Care of the Older Person conditions and the aetiology factors which contribute to the development of these pathophysiological processes.
2. Conduct a comprehensive, holistic and systematic nursing assessment of the older person with consideration to physical, psychological and social care needs including acute changes in condition and develop a patient specific nursing care plans to meet needs effectively utilising MDT supports.
3. Recognise and distinguish acute health alterations from chronic health conditions by utilising appropriate assessment and escalation tools, protocols, guidelines and pathways in order to ensure timely and effective care delivery
4. Professionally and ethically engage with the multidisciplinary team to advance discharge planning for rehabilitation, reintegration into the community, social inclusion, long term care and end of life care planning.
5. Develop, extend and confer specialist knowledge by sourcing evidenced based research and imparting through the written and verbal medium.

Indicative Module Content:

Nursing assessment and management of older persons.

Physical and psychosocial support of older persons.

Evidence based theoretical knowledge of aged related altered health including chronic illness, frailty, altered cognition and general functional decline.

Learning Requirements:

Registered General Nurse with minimum 6 months recent clinical practice experience. A nurse may be considered who is less than 6 months post registration if they have previous clinical experience in care of the older person. The nurse must be employed in a clinical area caring for older persons in the Mater Misericordiae University Hospital or affiliated institution. Students are required to complete a minimum of 75 hours clinical experience per month for the duration of the module (20 weeks).

How will my Feedback be Delivered?

Students shall receive feedback on all assessments associated with the Care of Older Person Nursing CPD programme. The purpose of feedback is the promotion of learning for the student. Feedback should be clear, understandable and be structured around goals, criteria and standards. Feedback can be in oral or written form, formal or informal. Students are responsible for engaging with feedback and applying consideration to the feedback given on their work. Feedback shall be given no later than twenty working days after the deadline for assessment submission, except work that is submitted late.
