The module will give the nurse an in-depth view of needs of older people in the context of managing co-morbidities and frailty. The nurse will have an understanding of the management of symptoms associated with co-morbid conditions and advancing age. It will also explore how the principles of Palliative Care can benefit those with life-limiting conditions.

About this Module

Learning Outcomes:

On completion the candidate will be able to:
• Describe the concept of frailty in ageing adults and differentiate the various methods for identifying frailty in ageing adults.
• Identify the relevance and importance of the assessment of frailty in healthcare settings.
• Apply the principles of comprehensive geriatric assessment (CGA) to the management of frailty and geriatric syndromes.
• Critically discuss the impact of advancing disease on the individual, the family, on caregivers, and on society.
• Critically evaluate a range of contemporary non-pharmacological and pharmacological and approaches to managing co-morbidities.
• Identify a range of evidence-based interventions to support family/non-professional caregivers.
• Implement nurse facilitated discharge planning for discharge to home with community supports (CIT rehabilitation) or to a long-term residential care setting.
